Chapter 885: Chapter 426: People Matter More Than Money, Malignant Tumors Triggered by Phlebitis

This is a great opportunity, and naturally, Director Lin wants to seize it firmly.

"There’s no problem with the consultation, but my skills are limited, and I may not be of much help. I will do my best."

Li Jingsheng readily agreed.

After finishing the meal, Li Jingsheng looked apologetically at Ou Qinglan.

"Qinglan, I’ll come to find you after work later! I need to go to the vascular surgery department now to diagnose the patient with Director Lin."

"Mm-hmm, remember to come to my office tonight!" Ou Qinglan’s eyes showed a hint of resentment, but in front of Director Lin, she behaved quite appropriately.

After all, being a wealthy heiress, she doesn’t need anyone to teach her to maintain her boyfriend’s dignity.

She has never embarrassed Li Jingsheng in front of everyone.

Followed Director Lin to the vascular surgery department.

Under him, there was just one doctor, a young man who was dark and skinny. There seemed to be only one nurse too.

The state of the vascular surgery department is rather dire.

With only one nurse, how do they manage shifts?

Li Jingsheng heard of a method called ’parasiting’. The department lacks revenue, with only one nurse, making it impossible to manage shifts normally.

What to do?

Spend some money, and attach to other departments.

Have others help look after patients.

It’s thanks to Dean Fu’s diligent efforts and long-term vision for a strong second hospital, that allows ’sesame-sized’ departments like endocrinology and vascular surgery to exist.

If it were a typical large hospital, they would have merged departments a long time ago.

"Dr. Li, you’re here!"

The patient, upon seeing Li Jingsheng, immediately became agitated as if seeing a loved one.

The woman accompanying him by the bedside should be his wife, wearing glasses and dressed like a workplace ’old ox’.

This woman probably works very hard at her company.

Many career women adorn themselves to be exquisite and pretty, but her hair was slightly disheveled.

Beneath the large black-framed glasses, her eyes appeared tired, with rather severe dark circles.

Such long-term pigmentation doesn’t form overnight.

It accumulates over long periods of overtime work, resulting in such severe dark circles.

Seeing the doctors enter, the woman immediately stood up, showing a placating smile to Li Jingsheng and Director Lin.

"Is the situation getting better?"

Li Jingsheng inquired about the patient.

"After the IV, it doesn’t hurt that much. Do I need surgery for this? If possible, I’d prefer not to do surgery so I can be discharged sooner. My wife is already very busy with her work, and now she has to take out time to take care of me."

The patient shared his difficulties and thoughts with Li Jingsheng.

Director Lin stood aside, feeling somewhat awkward.

He was supposed to be the attending physician.

The patient’s thoughts should be reported to him.

"Don’t worry about discharge just yet; we need to cure your illness before you leave. Otherwise, if you go home and something goes wrong again, you’ll have to be hospitalized once more. And you know, there are many clots in your veins, moving within the blood vessels, which is very dangerous. From your examination results, luckily the clots haven’t entered the arteries, otherwise your life could be at risk.

The blood vessels in the human body are interconnected; the problem with your right hand is because clots from the lower limb traveled to the hand.

If they move to the cardiovascular system, it results in a heart attack.

Entering the cerebral vessels would result in a stroke.

Deep vein thrombosis is most likely to enter the pulmonary vessels, causing pulmonary artery or vein embolism, all of which can directly threaten your life."

Li Jingsheng stated very seriously.

Understanding the patient’s concern about hospital expenses and eagerness to be discharged.

But when it gets to this point, the disease must be treated.

The patient doesn’t know the dangers of this illness; it is Li Jingsheng’s duty to provide the necessary knowledge.

"It’s that serious? Honey, listen to the doctor, we’ll do whatever it takes to get you well, even if it means transferring our child to a regular school next year."

The woman was so frightened by the severity of the illness that her face turned pale.

She immediately committed to helping her husband get well.

This is actually the correct perspective.

Some parents always place all their hopes on their children’s upbringing, which isn’t good.

If they themselves are not successful, they hope to ensure their children are successful and excel, which is a good intention, but spending hundreds of thousands a year might end up sending the children overseas for studies.

But once they return, their monthly salary might not even surpass that of a regular graduate.

The psychological gap would be tremendous.

Moreover, the pressure the children endure is also enormous.

"As for whether you need surgery, that’s up to Director Lin to decide. He is a vascular surgery expert and your attending doctor; he understands the disease better than anyone."

Li Jingsheng handles human affairs better than the patient.

It would naturally be inappropriate to embarrass Director Lin.

He knows who is in charge.

He will not overshadow the host.

"Okay, we will definitely follow your and Director Lin’s advice. Please, save my husband; our child and I both need him, we cannot afford to lose him."

The woman was still somewhat worried about her husband’s condition, continually pleading with the two doctors.

"Rest assured, as long as he can be saved, we will do our utmost. Your cooperation is also crucial."

Director Lin gave a preliminary assurance.

Doctors, especially those at the level of attending physicians, are always cautious with their words. They won’t say anything like a guarantee of full recovery.

After all, doctors are human too, and can only do their best.

"Let me check your left leg and right hand again!"

Li Jingsheng came with a mission this time; otherwise, who would chat with patients in the ward?

No doctor has that kind of leisure.

He felt the patient’s left leg, especially the calf area, where the cord-like hard lumps had disappeared. This indicated that the medication had significantly dissolved the thrombus.

He sensed that the skin surface around the patient’s right hand vein was still a bit warm.

This suggests a high likelihood of localized inflammation.

Most likely it is phlebitis.

When Li Jingsheng examined the patient, the patient did not cry out in pain this time. However, when touched on the larger veins, the patient still showed a painful expression.

Moreover, he noticed that the feel of the veins was still slightly different from normal veins.

After the examination, he left the ward with Director Lin.

"Let’s talk in the office!"

Director Lin invited Li Jingsheng to his office.

It was a small office, arranged in a very elegant manner.

Books and items were neatly organized, and it was clean.

"The office is a bit small, unlike Director Hua of your Orthopedics’ large office. Please, have a seat."

Director Lin made tea for him.

The cup was a disposable paper cup.

The cups used for hosting guests are generally like this.

Clinical doctors rarely brew tea and serve guests cup by cup with small cups.

Usually, a cup is brewed at once.

Deep paper cups are used for hosting.

"You’ve seen the patient’s condition just now; it’s still not optimistic. The venous thrombosis has been definitively diagnosed, and the patient’s vein has inflammatory reaction, which is also basically confirmed. He had just an ordinary fall injury, and developed such extensive venous thrombosis; I think it’s unreasonable by itself. There might very well be other unknown causes."

Though Director Lin might not have had great success in his career, he is quite competent.

He has deep insight when analyzing the causes.

"You mean starting with discussing phlebitis?"

Li Jingsheng asked.

"Sure."

Director Lin nodded.

Thrombophlebitis is not complicated and can be divided into two types in clinical practice.

One is benign superficial thrombophlebitis.

This is the most common.

It often manifests as inflammation of a single vein.

The causes of this kind of phlebitis are mostly limb trauma or varicose veins.

The patient does not have varicose veins, but there is the trigger of limb trauma.

In addition, there are many causes of phlebitis, such as localized purulent infections, intravenous injection of hypertonic glucose solutions, or other irritating drugs, etc.

Except for the limb trauma, there are no other obvious issues with the patient.

The second type is migratory thrombophlebitis.

This type is quite rare clinically.

However, this patient first had a left lower limb injury, and then, more than a month later, after the left lower limb pain, even the right hand followed with pain.

This suggests that the patient likely has migratory thrombosis.

Therefore, the patient meets both conditions.

This requires Li Jingsheng to make further diagnosis and differentiation.

Migratory thrombosis is common in middle-aged men, with the most prominent symptom being inflammatory responses in the superficial veins of the limbs, characterized by skin redness and heat, with scattered tender small nodules.

Performing a complete blood count might reveal a significant increase in white blood cells.

At the same time, the patient might experience fever.

Usually, it is not high fever, but rather low or moderate fever.

Li Jingsheng meticulously analyzed the patient’s condition, finding many corresponding aspects.

"Can I see the patient’s blood test results?"

"No problem, all the examination data is here, you can directly check it."

Director Lin stepped aside, allowing Li Jingsheng to sit at the computer and check.

He had logged into his personal doctor’s account.

The director’s permission level is quite high.

Especially for patients in their own department, they can easily view all the patient’s medical records.

Upon carefully reviewing the patient’s complete blood count, Li Jingsheng immediately noticed a significant increase in white blood cells.

This further corroborated the possibility of migratory thrombophlebitis.

There are numerous causes that can lead to migratory thrombophlebitis.

Clinically verified causes include gastric cancer, pancreatic cancer, lung cancer, etc.

The patient’s examinations are obviously insufficient.

Currently, only some basic tests and ultrasound examinations have been performed.

"Director Lin, I think there is a high possibility that this patient has migratory thrombophlebitis. There are many corresponding factors. However, the etiology of this disease is varied, with the most common being tumors. Considering the patient’s busy work, high life stress, in addition to substantial smoking and drinking, shouldn’t we conduct an abdominal CT scan to assess the situation further?"

Li Jingsheng proposed his diagnostic opinion.

Diagnosing such a complex case is really not an easy task.

Some related in-depth examinations must be supplemented.

The currently available diagnostic clues are extremely limited.

Who would have thought that a venous thrombosis caused by an injury might actually uncover a malignant tumor?
