Chapter 1452: Chapter 1452: Obstructive Cholangitis

(I Love Fruit Paradise, Sheep Seed, Clear Lotus Flowing Moon Ticket Encouragement)

Even though lunch was a bit late, everyone still enjoyed their meal.

Miao Rui put in some effort, and although he didn’t complete the surgery within twenty minutes, Liu Banxia generously added more dishes.

Encouragement should be given whenever it’s due. A few minutes off is just a few minutes off. Otherwise, Xu Yino and Liu Yiqing’s eyes would turn blue with envy, so we definitely need to add a dish for everyone.

Recently, it’s been quite exhausting as the collaboration between the two hospitals requires them to run around frequently, which is a real drain.

Especially since the emergency center’s work is so busy, proper rest isn’t easy to come by.

"Teacher Liu, when will the mentor’s wife arrange a meal with big prawns or something? I’m craving seafood," Xu Yino said.

"I’m puzzled. Do you really see my place as a canteen or something?" Liu Banxia asked, laughing and crying.

"The mentor’s wife has her sourcing channels. The seafood we eat outside is all farmed and doesn’t taste as good as hers," Xu Yino added.

"We could actually pitch in for some procurement costs. The key is, even if we go to a seafood restaurant, it’s rare to actually get genuine seafood. Right?"

"Don’t say Dad, you should say Mom," Liu Banxia shot him a look.

"Let’s arrange something later. The thing is, seafood doesn’t taste great when cold; it’s best enjoyed hot. And how should we get it here? We’d have to mix in some clams, crabs, scallops, and all that," Liu Banxia said.

"Hehe, just prepare anything, we’re not picky," Xu Yino replied with a grin.

"Why so happy? I was just saying," Liu Banxia said leisurely.

Xu Yino exchanged a glance with Liu Yiqing, they still need to keep at it.

"No worries, I’ll talk to the mentor’s wife later. Once she decides something, Teacher Liu won’t dare to argue," Liu Yiqing said casually.

"Calm down, calm down, what if Teacher Liu secretly nibbles on everything during the journey?" Xu Yino feigned bravado.

Liu Banxia felt speechless, knowing this was directed at him. But he couldn’t help it; the girls were right about their close ties with Qiaoqiao.

Strolling back to the emergency center, the group dispersed to continue attending to patients.

Joking is joking, work needs to be done. One’s primary responsibilities can’t be neglected.

"I’ve noticed you’re starting to humor this little group, aren’t you?" Wei Yuan asked with a smile.

"You have to humor them; they’re treasures. Whether my future days are relaxed or backbreaking depends on them," Liu Banxia replied.

"They’re quite a handful, even thinking about seafood now. Later, we’ll let them go have fun by the seaside to eat and relax. That’s the only option," Liu Banxia commented.

Listening to Liu Banxia, Wei Yuan felt deeply moved.

Their mentor-student relationship is quite unique, perhaps because Liu Banxia is a special person. They can’t be seen making such a ruckus in hospitals.

However, despite the antics, their relationship remains harmonious.

Furthermore, all six of them handle medical issues with extreme precision, showcasing the influence of Liu Banxia’s methods.

Liu Banxia’s personality is quite like this, even Su Wenhao seems to be gravitating towards it now.

"How was your lecture at the medical school? Can you attract some pediatric surgery interns?" Wei Yuan asked.

"That’s hard to say. Regarding pediatric surgery, many students resist it. If not, it may require a gradual shift," Liu Banxia responded.

"The issue is we lack the conditions needed to attract them here. We’re somewhat low on the desirability scale. Plus, in this area, I lack the confidence to train them properly," Liu Banxia said.

"Have some faith in yourself, no problem. You handle many general surgery procedures expertly now," Wei Yuan noted.

"Please don’t praise me. Which direction do you plan to delve into for research? After Boss Shi finds stability, you should be next," Liu Banxia asked.

"Sigh... discussing this, I’m indeed troubled. Initially, there wasn’t such a big ambition; now, motivated by you guys, I want to make an effort," Wei Yuan explained.

"Actually, it’s not a big deal. Your skills are evident, just work hard," Liu Banxia said.

"For the emergency center to be recognized nationally, we need many elements. Eventually, we’ll likely invite some experts from outside. If we’re not diligent, we risk falling behind," Liu Banxia advised.

"Regardless of the area, if you need my help, just let me know. I’m committed to providing full support. Back when I was training, you all took such good care of me; I can’t stand on the sidelines now," Liu Banxia added.

"No need for formalities; I’ll definitely call on you when needed," Wei Yuan nodded.

In terms of seniority, he’s older than Liu Banxia. But in terms of achievements, he does lag behind Liu Banxia now.

Whether Liu Banxia can assist him or not, Wei Yuan is just pleased that Liu Banxia expresses such an attitude.

Because he knows Liu Banxia is straightforward, saying and doing just as he promises. In medical skills, Liu Banxia undoubtedly surpasses him significantly now.

Chatting casually with Wei Yuan for a while, Liu Banxia wandered around, wanting to see how these interns were doing.

This group is about to finish their internship soon; for them, it might just be considered a small part of the larger educational process. But in Liu Banxia’s heart? He takes it quite seriously.

This is a batch he personally oversaw; even if he doesn’t seek fame, he hopes they end with a good result.

"Teacher Liu, are you busy? If not, could you lend me a hand with a case I just got?"

Just as he was about to sit down to drink tea, Su Wenhao walked over.

"What’s the situation? Usually, you wouldn’t look for me unless it’s something big." Liu Banxia asked with a smile.

"The patient has upper abdominal pain, fever, jaundice, and is somewhat skinny. The ultrasound is not very clear, likely a bile duct issue. Then I ordered a CT and liver function tests," Su Wenhao explained.

"Are the results out?" Liu Banxia asked.

"The bilirubin levels are very high; the CT results aren’t out yet," Su Wenhao said.

"Teacher Liu, if it is really a bile duct issue, is surgery going to be very complicated? Judging by the patient’s condition, I suspect it may not be just simple cholangitis."

"Right now, it’s hard to predict; we’ll have to wait for the CT images to find out whether it’s a tumor or stones," Liu Banxia replied.

Based solely on Su Wenhao’s symptom description, Liu Banxia judged the patient’s bile duct condition wasn’t great, probably at least some obstruction.

Su Wenhao isn’t someone who exaggerates; if he says bilirubin levels are high, it usually signifies a serious problem.

The reason bilirubin levels are high is due to inflammation in the bile ducts; bilirubin doesn’t discharge smoothly, accumulates in the bile ducts and liver, and excess bilirubin flows back into the blood.

There are generally two reasons for impeded bile flow: either bile duct stones, causing obstruction due to excess accumulation, or, less favorable, it could be a tumor.

If it’s indeed a tumor? What Liu Banxia could foresee as consequences would be quite terrifying.

Benign tumors are rare here, and with the patient’s current condition, it’s likely metastasized. If it’s spread, the surgery might not be straightforward.

This is the main reason Su Wenhao sought a consultation with him, concerned it might be beyond his capabilities.

"Did the patient come on their own?" Liu Banxia asked.

"No, came with his wife," Su Wenhao replied quickly.

"Alright, let’s still have a look at the CT images later; we need to be cautious given the situation," Liu Banxia said.

"Teacher Liu, the judgment you made, isn’t it somewhat worrisome too?" Su Wenhao asked tentatively.

"Yes, if it’s bile duct stones, they would likely show clearly on the ultrasound; the CT is mainly to confirm," Liu Banxia said.

"But whatever we say now doesn’t matter; it still depends on the examination results. You’ll have to make the call later; I’ll be here watching."

"Teacher Liu..., can I do it?" Su Wenhao asked hesitantly.

"Why not? As long as you can interpret the CT scan, the rest involves communicating with the patient and their family," Liu Banxia said.

Su Wenhao nodded, knowing it’s also a test from Liu Banxia—testing his communication skill with patients and their families, which isn’t a light task.

After waiting a bit, Su Wenhao saw the patient and his wife walking over.

"Is the CT done?" Su Wenhao asked.

"Doctor, it’s done. They said the images would take some time to be ready; if you’re not busy, you can view them on the computer first," the patient said.

"Alright, let’s take a look; you can rest on the chair first," Su Wenhao said.

Liu Banxia also took a careful look at the patient; the jaundice was quite severe. This situation isn’t good, and might be heading towards a worse direction.

Su Wenhao brought up the patient’s CT on the computer and after a careful look felt a chill run through him.

No stones were seen on the images, significant mass detected.

This means the possibility of a tumor cannot be excluded.

"Doctor, what do you see?" the patient asked.

"Don’t worry yet, I need a thorough comparison," Su Wenhao tried to keep his voice steady.

Doctors are the guiding light for patients; especially with the patient’s current condition, bile duct cancer is a strong possibility.

"Are you still experiencing abdominal pain? Maybe lie down on the examination bed over there; it might help relieve it," Su Wenhao continued.

The patient thought for a moment, then moved to lie down on the side.

Su Wenhao breathed a sigh of relief, not having to face the patient directly anymore.
