Its head is enveloped in a shifting, distorting light mist, faintly revealing a stiff face riddled with fine cracks, like melted then solidified.

A murky eye, almost devoured by purple light, flickers within.

The mist seems merely to obscure the face.

This isn’t a pure void incarnation like the "Annihilation Throat".

This is a "Forgotten One".

A native of the world, devoured and twisted by the void.

Essentially, many void creatures are like this, but some are specifically called "Forgotten Ones".

Because those given this name are from the previous destroyed world, the void-corrupted lives.
