Chapter 1431: Chapter 1431: Orthopedics Expansion Is Imperative

"What a relief, what a relief, no mishaps, it’s really a pulmonary embolism."

After nearly twenty minutes, Xu Guosheng walked out of the emergency room, his forehead covered in sweat.

"Is it really a pulmonary embolism? Did you use the ultrasound to check again?" Liu Banxia asked.

Xu Guosheng nodded, "After our examination, we currently suspect it’s a deep vein thrombosis in the lower limbs, which dislodged and got stuck in the pulmonary vessels."

"The paramedic described the patient having cold symptoms. My personal judgment is that it’s a manifestation caused by a small embolus dislodging. The patient mistakenly judged it herself, she must have taken a lot of cold medicine, which of course had no effect."

"It is indeed a stroke of luck, this is experience. If the judgment was wrong, it could have been very dangerous." Liu Banxia said with emotion.

"I’m already drenched in sweat. My instincts were telling me there’s a high likelihood of pulmonary embolism in this patient. But performing thrombolysis is basically taking a risk." Xu Guosheng said with a wry smile.

"There’s been some effect for now, the blood oxygen level has improved, one could say she narrowly escaped death. We still need to contact the family quickly, and more detailed examinations are needed to find out what caused her deep vein thrombosis in the lower limbs."

"At this age, it’s really unlikely. Even if she’s been staying home all the time, she couldn’t be lying down all the time, right? It’s not like an occupational disease from a standing job, she looks like she’s just graduated."

Liu Banxia nodded in understanding, as he had also treated several patients with deep vein thrombosis of the lower limbs. Naturally, he knew that this condition is very dangerous and can easily cause pulmonary embolism.

The formation conditions for deep vein thrombosis in the lower limbs are basically three types: venous blood flow stasis, venous wall damage, and a hypercoagulable state of blood.

Venous blood flow stasis is the most common of these three causes and is prevalent as an occupational disease in many professions.

For instance, those who work standing up frequently, drivers who need to drive for long periods, and those who require bed rest are all prone to forming deep vein thrombosis in the lower limbs.

Additionally, some trauma patients and those needing surgery are also at risk groups for deep vein thrombosis in the lower limbs.

All involve long periods of sluggish venous blood flow in the lower limbs, and the longer the time, the greater the likelihood becomes.

However, this patient, from her external condition, doesn’t seem to have possibilities related to these causes.

The current emergency treatment for her is merely addressing the symptoms. If her deep vein thrombosis in the lower limbs isn’t dealt with, or the cause isn’t found, she might again be in danger in the future.

As for venous wall damage, unless she has a history of prolonged intravenous injection or has had fractures. A hypercoagulable state of blood? Mostly acquired by those undergoing major surgeries or end-stage tumor patients, which she doesn’t seem to resemble.

This is a dangerous situation; it’s not just about emergency aid and temporarily keeping the patient out of danger.

However, Liu Banxia doesn’t need to concern himself with this patient’s situation for now. If her thrombus is too large and requires surgical removal, that’s when it becomes the general surgery’s responsibility.

After wrapping up this issue, he looked at Zhou Shuwen’s office with a bit of a headache.

There’s no use in worrying, he still has to go. He has to tackle the issues that need addressing, whenever problems arise, he must bravely shoulder the responsibility.

He knocked gently on the door a few times and carefully opened it, stepping inside.

"Looks like there’s something up, otherwise you wouldn’t be this cautious, your pace entering the door has slowed significantly." Zhou Shuwen said with a smile.

"Director, there are indeed some minor issues. Currently, the development of the orthopedics department has hit a bottleneck. Is it possible to consider putting the expansion of the orthopedic department on the agenda first?" Liu Banxia tentatively asked.

"Are the wards already at full capacity?"

Working in the hospital for years, Zhou Shuwen directly hit the crux of the matter.

"Yes, the turnover rate for orthopedic patients is slow. Previously, we only handled emergency patients, so it was manageable. Now there are some elective patients, which is becoming difficult to manage." Liu Banxia quickly said.

"In that case, preparations indeed need to be made early." Zhou Shuwen nodded.

"Orthopedic patients have their own specialties, and our emergency center’s development should consider a balanced approach. Later, submit an application and see what the budget should be."

"If it’s not much, we can get this done as soon as possible. But I’m quite curious, why is it you bringing up this matter?"

Liu Banxia grinned, "They plotted against me. Plus, now both the hospital and emergency center are leaning towards pediatric construction, so this matter fell on my shoulders."

"If we really expand the orthopedics department, apart from the arrangements of the wards, we’ll also need some nurses and doctors. Right now, they are rather strained; even Teacher Wang Bo often has to be on the front line."

"I can’t help with that. You can discuss the nursing issue with Zhou Li. For doctors, if hiring externally, it will also be on a departmental contract." Zhou Shuwen said.

"The hospital’s policy bias towards our emergency center is already significant. Besides the pediatric department receiving some attention, other departments are struggling."

"You can discuss this with Xuehai, both of you can collaborate with orthopedics to see how many people can be minimized while still functioning."

"In relative terms, recruiting orthopedic doctors is a bit easier. It’s best to create some flexibility, perhaps selecting some people from the interns and resident doctors we’ve trained as reserve forces."

"Orthopedics is also an important department, and the selection of personnel requires a strict review. Because a doctor with poor skills might cause greater harm to the patients."

"For now, you temporarily take charge of the support in this area. In our emergency center, currently, you are the only one with such capability. Is there anything else?"

Liu Banxia shook his head, "I feel like I’ve fallen into another pit."

"Haha, I know you’re very busy, but orthopedics is also a department you need to understand and master," Zhou Shuwen said cheerfully.

"Alright, I’ll go find Old Chen. Now I’m on an official mission to find him, so he won’t avoid me when he sees me," Liu Banxia said.

Zhou Shuwen nodded and continued with the work at hand.

Walking out, Liu Banxia felt the burden on his shoulders grow heavier.

Currently, he has a lot of work in his primary department, general surgery, and he also needs to support hospitals in Jinshui District and his hometown. He has to ponder over pediatric surgery and also worry about orthopedic matters.

He truly feels a sense of being unable to do everything at once, but he also knows that this is an opportunity to enrich his work experience.

Orthopedics is a major department, and if he can participate in the expansion of orthopedics this time, and even get involved in the selection of doctors, it will be real management experience.

In terms of support, the actual patient contact won’t be much. If the orthopedic department were too busy, it should have been him supporting it anyway.

"Wow, you came out so quickly. Did the director approve it?" Wang Chao asked curiously.

"He approved it, but we need to make a plan first. We also need to coordinate with orthopedics on the required number of medical staff," Liu Banxia said.

"Impressive, I thought it would be postponed for a few days," Wang Chao said.

"That’s why our director is impressive; he immediately pinpointed the key issue. He also knows the orthopedic department’s problems need rapid treatment, but still tries to compress the funding as much as possible," Liu Banxia said.

"You better go and work on that. I think Director Chen seems to be free now," Wang Chao hurriedly said.

That’s also a big deal; he knows the situation is fine, he doesn’t need to dive into too much detail. Otherwise, it is not just gathering information, but stirring up trouble.

"Hide, keep hiding. Why aren’t you hiding now?"

Reaching the cardiovascular office, Liu Banxia crossed his arms.

"Hide what? Why should I hide? Do you need something? I’m working on a surgical plan," Chen Xuehai said earnestly.

"The director told me to discuss orthopedic matters with you. I’m clueless here; what do you think we should do?" Liu Banxia asked.

"It’s quite simple. We just need to negotiate with the orthopedic department," Chen Xuehai said.

"They surely hope for more people, and we should compress as much as possible. Anyway, no department ever thinks they have too many people; isn’t that the jist of it?"

"What? Is that how you perform as the deputy director of operations?" Liu Banxia asked curiously.

"Otherwise, how? I don’t believe when you face this situation in general surgery, you’d graciously say we’re fine now, feel free to expand without issues," Chen Xuehai said.

Liu Banxia scratched his scalp, realizing that’s exactly the situation.

Currently, the emergency center’s general surgery is somewhat special because of the six rising stars. Otherwise, with this large patient intake, he’d be jumping to demand more staffing.

This also indirectly reflects the importance of the six rising stars; they truly support half the sky of general surgery.

Liu Banxia repeated Zhou Shuwen’s guidance to Chen Xuehai, establishing the overall strategy.

With this big direction, the following tasks become somewhat easier to handle.

However, both know today’s work will be busy.

Reviewing floors, confronting orthopedics, confirming timelines and recruitment progress, they must solidify and execute the tasks well.

Previously, the hospital helped arrange things, so many tasks didn’t require attention.

Now, it’s different, involving many interests and relationships, whether in construction or personnel recruitment. A slight misstep can lead to trouble.

Being taken advantage of by the emergency center’s colleagues is no big deal; being tricked by outsiders, however, makes life difficult later.

Anything involving interests demands high vigilance. Many stumble over interests, intentional or not, it’s easy to make mistakes.
