The Enhanced Doctor
Chapter 1449: Respecting the Patient’s Choice
Chapter 1449: Chapter 1449: Respecting the Patient’s Choice
"Teacher Liu, the patient’s CT results are out, but they’re a bit puzzling and unclear."
Miao Rui approached Liu Banxia.
Liu Banxia frowned. The fluid accumulation in the abdomen of the car accident patient seemed substantial; there should be internal bleeding. Why hasn’t the CT shown anything?
After carefully examining the CT images on the computer, it turned out to be true.
"Teacher Liu, could it be that the patient originally had ascites?" Miao Rui asked.
Liu Banxia shook his head, "The likelihood is very low. He’s not very old, and his liver shape is good. I suspect it might be mesenteric bleeding, just that the blood coagulated during transportation."
"Although it’s rare, considering his situation, it’s very possible. Let’s suggest an exploratory laparoscopy to the patient; it’s a safer option."
He must make the decision about this matter.
Exploratory laparoscopy is also an interventional examination, not something Miao Rui can decide.
But the patient’s situation is indeed peculiar, and he hasn’t experienced this before. Yet summing up the fluid as ascites also seems wrong.
He’s encountered ascites cases before, and this patient doesn’t fit the profile at all.
Miao Rui nodded, considering Liu Banxia’s instructions as confirmation of the need for exploration.
"Director Liu, may I ask you a question?"
Wang Chao came over.
"What are you up to? Planning to study NOSES surgery? It’s fine, you can first chat with Xu Yino and the others for a preliminary understanding," Liu Banxia said casually.
"Director Liu, please be serious," Wang Chao said earnestly.
"So, what are you really up to?" Liu Banxia asked.
"Haha, I overheard something and wanted to ask if I could have a little holiday when I get married," Wang Chao said.
"You know how tough my job as a chief resident is. We don’t plan to travel far, just wander around the South as we’ve never been there."
Liu Banxia rolled his eyes, "I thought it was something important, just this? Just let us know in advance when the time comes."
"But chances are low that many from our side will attend your wedding. You know how things are here; it’s truly difficult to have so many people leave at once."
"I understand. We didn’t plan to invite outsiders anyway, just close relatives from both sides to meet and mingle," Wang Chao said.
"I’m happy you’ll approve a long holiday. It’s because you set a bad example, getting married without taking leave or honeymooning, which isn’t good."
"I had no choice back then. Anyway, I’ll make it up to Qiaoqiao later; maybe we’ll go on a family honeymoon," Liu Banxia said.
"Things that need to be arranged should be arranged; we can’t leave regrets. Do you have anything going on now? Let’s chat about some minor issues."
"Are you trying to trick me into requesting extra dishes?" Wang Chao asked cautiously.
"Am I that petty? I’ll add some for you at lunch," Liu Banxia said helplessly.
"Oh dear, oh dear, it’s really serious this time. You voluntarily offered extra dishes; it must be a big issue," Wang Chao said with a gloomy face.
Liu Banxia ignored him and directly pulled him into the office.
"So, what’s the matter?" Wang Chao asked.
"It’s nothing much. I had a chat with Old Qiu yesterday and did some self-reflection. I feel I’ve been a bit too aggressive lately. Do you have a similar feeling?" Liu Banxia asked.
Wang Chao shook his head, "I don’t have that feeling. Anyway, working with you is definitely more exhausting because of your fast pace."
"It’s not that you have overly high standards for everyone, forcing them to work tirelessly. It’s mainly because of your rapid growth, which naturally makes everyone feel pressured."
"Now, with your upcoming support work in Jinshui District Hospital and your hometown’s hospital, everyone knows you’re busy, so they have to work harder."
"I don’t see anything wrong; this is a normal situation considering your growth has been fast. But why do you have such thoughts?"
"It’s nothing major; I gave a speech at the medical school yesterday and started to feel like I was cajoling. Fortunately, Old Chen helped counterbalance it, urging students to consider carefully," Liu Banxia said.
"Although I used to say similar things, it was more in a joking manner. Even when telling students it’s tough to study at our emergency center, I mainly emphasized our achievements."
"Now that I think about it, including those I currently mentor, there might be people misled by my words. Honestly, my real intention is for these people to come knowing they will face challenges."
Wang Chao rolled his eyes, "I thought it was something important, but this is nothing at all. Compared to TV ads, our promotion is very modest."
"I think you don’t need to worry about this issue. Just let it be. Anyway, our reputation at the emergency center is getting better and better. I believe more people will want to intern here in the future."
"But the pediatric surgery you’re all working on is indeed a tough nut to crack. It’s not easy. Our batch went through the same thing."
"Sigh... I know it’s not easy, but we still have to keep at it," Liu Banxia sighed.
"Teacher Liu, um... can we just observe the patient’s condition for now?"
Miao Rui walked in at this time.
Liu Banxia frowned, "Is the patient very resistant to the laparoscopy?"
"Yes, he said he doesn’t want to endure the pain or spend the money. He’s already got a puncture in his chest, and it would be too much to have another one in his abdomen," Miao Rui said.
"Observation, huh? The risk of just observing is too high. The main issue is that there’s quite a bit of fluid in his abdominal cavity. I’ll go have a look with you," Liu Banxia said.
This is indeed a problem. A laparoscopy involves quite a lot of expenses. I should have gone with Miao Rui in the first place instead of being preoccupied with other thoughts.
They arrived at the patient’s bedside, and his family was present as well.
"Doctor, thank you. The imaging said you aligned my bones properly,"
the patient said when he saw Liu Banxia.
"In your current situation, I’m actually more worried about the fluid in your abdominal cavity than your fracture. Are you still in pain?" Liu Banxia asked.
"No, I’m not really feeling the pain now. But to say it’s not painful at all isn’t realistic, considering my injury is not light," the patient said.
"Can’t we just skip the puncture in the abdomen? This doctor just explained everything to us. The opposing car had no insurance, not even compulsory traffic insurance. I don’t even know if I can get the medical fees back,"
"That’s definitely a problem, but I think you shouldn’t worry about it for now. Even if they don’t want to pay, there’s still the legal system. You can sue them," Liu Banxia said.
"The fluid in your abdominal cavity is worrisome to me. I suspect that it’s blood from a ruptured blood vessel due to the impact."
"During the transfer, for some reason, the rupture clotted successfully. But this kind of clotting isn’t very safe; it might form some thrombus."
"Although we can take a conservative approach and just observe, the risk is really high. I recommend doing the laparoscopy, not to earn the examination fee."
"Because if your condition continues to worsen, we might have to perform an open surgery. That would be much more invasive and could delay the emergency response."
"Well... Doctor, I think I’m fine. If I feel uncomfortable, I can just call you, can’t I?" the patient said with a smile.
"You see, I feel quite energetic now. People say I’m really lucky to have survived. Anyway, I don’t think it’s a big deal, so let’s avoid the trouble. After all, it’d require general anesthesia, right?"
"Alright, we just need to make everything clear," Liu Banxia said.
"Even if you don’t undergo the laparoscopy, it doesn’t mean it’s unnecessary; we just need to respect the patient’s choice."
"But personally, I really advise you to reconsider. Your situation seems a bit special, and there’s indeed quite a bit of fluid in your abdomen."
"How about this, think it over and discuss it with your family. If you decide to do the examination, just call a nurse to notify us."
"Remember, if you feel any discomfort, especially in the abdomen or head, make sure to call the doctor. Your situation, if dangerous, can really be dangerous."
"Don’t think we are trying to scare you. We’ve seen too many cases. If your abdominal cavity really has other issues, you can imagine the risks."
"If we do the examination, we only need to make a small incision. If there are other issues, we might have to open up your entire abdomen."
As he spoke, Liu Banxia gestured with his hand.
"Okay, if I feel anything’s wrong, I’ll call you," the patient said.
Liu Banxia felt helpless, the patient’s attitude was too firm, and even he couldn’t persuade him.
Even though the patient said this, Liu Banxia could tell he was just being dismissive.
"Miao Rui, keep a close watch, and check in every half hour," Liu Banxia said.
Miao Rui nodded, saying nothing.
This is indeed respecting the patient’s decision, though he hopes the patient agrees to the laparoscopy. But it’s not what he wants; without the patient’s signature, everything’s moot.
The initiative is always in the patient’s hands.
